Questions
Is this the same as a variation order?
Yes. Extra work, variation and change order all mean work that was not in the original quote.
Does the client have to sign it?
It is not required by law, but a signed price agreed before you start is the strongest proof you have if they argue later.
What if the client is not on site?
Send it as a photo and ask them to reply "agreed" in writing - or send it from the app for them to sign on their phone.
